Making request for an EHC Needs Assessment

This link provides you with a template to request the local authority carry out an EHC Needs Assessment for your child. The letter includes all the necessary legal points, and you can simply amend the sections with your personal details.

Tip: Always send the letter via email and request a read receipt if possible. This will not only ensure you have a paper trail of your application, but also provide a date stamp confirming when the local authority received your request.

Request a re-assessment of an EHC Plan
